Instructions for downloading order form on a MAC:

1. Click on the form below and the Downloads box

    will show up

2. Click clear and then on the red button to close

    this box

3. Double click on the form which is now on your

    desktop

4. Fill out the form and SAVE

5. Email form as an attachment to:

Instructions for downloading order form on a PC:

1. Click on the form below and the form will open.

2. Then click on  File at the top of the page, then

    save as.

3. Then click the ok in the box, and save the file.

4. Then open the form fill it out and Save it again.

5. Email form as an attachment to:

If you are unable to download the form, email Deb and I will try to talk you through it. If you do not have MS Word Please email Deb and a copy of the form will be sent to you.
